The goal of Supported Living is to ensure that young people are supported whilst growing into their independence.

Our Aims

Our aim is to provide a warm, safe, and caring environment for vulnerable young people aged between 16 and 18 years of age who are leaving care either from residential or fostering environment into our semi-independent service.

Our Vision

ACH Healthcare Limited was founded in 2017 as a staff supply and Domiciliary Care Agency and to date, has established itself as a recognised provider of quality staff within the Supported Living Sector. Our vision to diversify our services as an agency to providing our own Supported Living accommodations is taking shape and we have acquired our first two accommodations for Young People and Adults in Milton Keynes, Bucks and Takeley, Essex.

Our Aims

Our aim is to provide a warm, safe, and caring environment for vulnerable young people aged between 16 and 18 years of age who are leaving care either from residential or fostering environment into our semi-independent service. We will enable the young persons to develop their skills towards independence whilst they are growing into young adults. These young people may have previously experienced emotional pain, social and academic failure, and rejection.

ACH Healthcare endeavours to enhance Young People’s well-being into a bright and positive future by providing a recovery model service which enables the Young Person as well as Social Care stakeholders to observe the progress of the Young Person through their diminishing need for support by using a three-tier step-down structure in our accommodation offering:

• Intensive Support
• Intermediate Support
• Semi-Independent Living

With this three-tier structure it is our goal to provide Young People with continuity of care through familiarity and a dedicated Support Work Team working through the three-tier structure.

On arrival, a Key Worker will undertake detailed discussions with the young person to address any presenting problems, identify their needs and gain an understanding of what they like and don’t like. Our aim is to empower the young with transitional skills into adulthood.

Adults and Young People will be supported in separate properties based on their levels of need.

Our Staff

ACH Healthcare places great importance on recruiting and retaining high calibre staff that possess appropriate qualifications and experience. We aim to appoint a staff team of mixed ages, gender who will individually and collectively present as positive role
models for the young people we support.

All staff undergo the standard Mandatory Care Training, Medication Administration as well as Prevention and Management of Violence and Aggression training which means they are equipped to deal with difficult and/or violent situations. 

Expected outcomes for the people we support

At ACH Healthcare, we believe that wherever possible, young and vulnerable people benefit from living within a family. We recognise that for some, this is not always possible and that living in a semi-independent accommodation can be the most appropriate alternative.

Therefore, within our facility we aim to achieve the following outcomes for the people we support:

To develop person-centred pathway support packages for the people we support by involving them and where appropriate their family in making decisions about how they are supported.

To empowering the people we support to learn, grow and make informed choices about their future by working with their key workers and other professionals so they can take time to repair and move forward from bad experiences in their life.

To enable the people we support to develop skills to communicate, interact with others and to integrate and contribute via discussions and meetings which allows their views and opinions to be heard regarding the delivery and development of the service.

To promote safety for the people we support by ensuring that each person has a holistic risk assessment and there is zero tolerance of discrimination, bullying or abuse of any type.

To promote the right culture within the facility that is conducive to learning, education, training and employment by ensuring resources and support are available the people we support.

To create an ethos and atmosphere that is conducive to personal growth by keeping the people we support involved within a safe internal environment and the wider community whilst taking all reasonable measures to minimise any risk of harm to self or others.

To promote respect and dignity for everyone regardless of their race, age, gender, cultural background, or ability.

To provide a caring, nurturing and stimulating environment that enables the people we support to reach their potential, through understanding and drawing on their personal strengths whilst celebrating their individual achievement as well as supporting them to build on their weaknesses and overcome fears.

To support and to maintain positive relationships in the lives of the people we support which enables them to keep their identity, self- respect, and care for their own wellbeing whether that be with family, friends or Key Workers or other professionals.

To enable the people we support to maintain their health and emotional needs by supporting them with GP, dental, optician visits, as well as monitoring their moods and general wellbeing in relation to their overall health.

To maintain a safe, supportive environment free from abuse in its many forms they feel supported and thrive.

To provide a framework of support which includes key workers and professionals in which the people we support can be brought to confront the meaning and significance of their behaviours thereby opening the possibility of change.

To prepare the people we support so they can eventually reintegrate into the wider community by consistently promoting trust and independence within the facility.

To work in partnership with parents, relevant adults, education, occupational health, and other professionals to ensure the people we support are enabled to maximise their potential.

By keeping abreast of best practice in Semi-Independent Living, aim to maintain a high quality yet cost-effective services delivered around the needs of the people we support.
